REGISTRATION INSTRUCTIONS:
Step 1.  Have the following information ready:
Emergency contact information, the Jersey and Short Size (Please see Size Chart).
PLEASE BRING A COPY OF YOUR CHILDS BIRTH CERTIFICATE TO
THE MANDATORY KIDS COMBINE.

Step 2. Online Registration! Click "Register" at the top of this page.
YOUR CHILD WILL NOT BE REGISTERED UNTIL YOU HAVE A COMPLETED
THE PAYMENT VIA VISA/MASTERCARD.
PARENTS: YOU MUST CREATE A PARENT ACCOUNT BEFORE BEING
TO REGISTER YOUR CHILD

If you would like to register in person, we will be registering participants
at PSN Park 7601 NW 176th St. Hialeah, FL on August 10, August 17th and August 24 from 12pm - 2pm)
REMEMBER TO BRING IN A COPY OF YOUR CHILDS BIRTH CERTIFICATE!!!!
(NO REFUNDS WILL BE ISSUED AFTER CHILD HAS BEEN ASSIGNED TO A TEAM)
